What the ‘health declaration’ to enter Hong Kong Disneyland looks like

Those entering Hong Kong Disneyland will need to make a health declaration when making online reservations. The park will be reopening on June 18 with limited capacity and new health and safety measures.

One of the new measures being taken when applying for reservations for the park is a health declaration.

The declaration also acts as Personal Information Collection Statement and the person filling it out will need to agree to it for themselves and those in their party. The guests agree that they do not have a fever, do not have respiratory symptoms, have not traveled outside of Hong Kong in the past 14 days, have not been in close contact with any confirmed cases of COVID-19 in the past 14 days and is not under a compulsory quarantine.

At Shanghai Disneyland people need government-provided QR Health Codes to enter. In the U.S., Disney has not yet announced what its reservation system will look like for Disney World or Disneyland, or if a liability waiver may need to be signed or agreed upon when making a reservation. Disney has placed on its Disneyland and Disney World website the following disclaimer: “By visiting Walt Disney World, you voluntarily assume all risks related exposure to COVID-19.”
